Eligibility

Any student resident in the Commonwealth with special educational needs in regard to vision, as determined by TEAM decision, ages 3 to 22 (or to the age a high school diploma is attained, i.e. college students are not eligible) can be eligible for services. Districts wishing to access the AIM Library must coordinate all paperwork and requests for materials through a Licensed Teacher of the Visually Impaired (TVI). The specialized instructional materials available can only be assessed, deemed essential, and administered by a TVI.

Once a local special education team evaluation has been completed, and special needs in regard to vision are written into the student’s I.E.P, a TVI may submit the required documents to register the student with the AIM Library.

For the purposes of State and Federal Funding, 2 documents must be filed with the AIM Library as evidence of the student’s educational program and eligibility.

Assurances Form

The Accessible Instructional Materials (AIM) Library requires the assurance that appropriate assessments, current medical records, and a written education plan are on file where the student attends school. The actual assessments, reports and plans are not to be submitted to the Accessible Instructional Materials Library.

By signing the Assurances Form the LEA agrees to have the assessments, medical records and written education plans on file in the event that any audit requires evidence of the student’s educational program.

The Assurances Form must be signed by the Special Education Director and submitted each school year for every student seeking to borrow materials. Original Assurances Forms (faxes and copies will not be accepted) should be mailed to:

To assist you with filling out the Assurances Form we offer our optional Medical Addendum. This form can be sent to medical offices for further clarification when an eye report or medical report has no acuities/field measurements, as we require either MDB, FDB, or NO to be indicated on our Assurances Form.

The Medical Addendum doesn’t need to be sent to the AIM Library. It was designed to help LEAs obtain further clarification from the doctor’s office. It will also help to determine whether a student meets the Federal guidelines of legal blindness in order to be counted in the Federal Quote Program.

Parental Consent Form

To be signed by parent or guardian to allow the AIM Library to exchange pertinent information with the Massachusetts Commission for the Blind, American Printing House for the Blind, Learning Ally, and the Massachusetts Department of Elementary and Secondary Education.
