Registration

Eligibility

Any student resident in the Commonwealth with special educational needs in regard to vision, as determined by TEAM decision, aged pre-K to 22 (or to the age a high school diploma is attained, i.e. college students are not eligible) can be eligible for services. Districts wishing to access the AIM Library must coordinate all paperwork and requests for materials through a Licensed Teacher of the Visually Impaired (TVI). The specialized instructional materials available can only be assessed, deemed essential, and administered by a TVI.

It is suggested that all forms be copied for your files prior to submitting them to the AIM Library for your records in case of later verification.

Assurance Form (AF)

AIM Library student registration requires the assurance that appropriate assessments, when applicable, current medical records, and a written education plan are on file where the student attends school. The actual assessments, reports, and plans are not to be submitted to the Accessible Instructional Materials Library but need to be on file at the school district. Original, wet signature Assurances Forms by LEA’s Special Education Director are required every school year for every BVI student. Assurance Forms are to be submitted during the school year they are covering. June and July submitted AF’s will cover the end of the school year, summer programs, ESY, and camp materials support. The window to submit AF’s for the school year opens on August 1st each year. Please do not email AF’s as it does not help expedite the process.

Consent Form (CF)

Consent to Release Information Form is to be signed by parent, guardian, or student over 18 managing their educational rights, to allow the AIM Library to exchange pertinent information with the Massachusetts Commission for the Blind, American Printing House for the Blind, and the Massachusetts Department of Elementary and Secondary Education.
